Nekrašas Volodkavičius (Nyekrasch Volodzkowycz), holder of Belica, donates the following to the Church of Garadzišča: three annual tithes of grain from the estates of Garadzišča, Dračylavas (Draczylowo) and Zaberdavas (Zabyerewo); a tithe from horse and cattle herd; four "saikas" (measurement) of honey; an inn exempt from taxes; a plot of arable land and two fields with meadows; 13 peasants for the sustenance of the Church of Assumption, St. Jacob, St. Andrew, St. Michael and St. Hedwiga, established by Nekrašas Volodkavičius in Garadzišča; and allows to fish in the waters of the estate of Garadzišča.

The attached seal is missing, except for the dark brown thread and 3 cuts.

The parchment is spotty, worn out, torn in places and decayed along the folds. Falsification.
Invocatio is written in enlarged letters. Initial is decorated and takes up 16 lines.
